We know that people with learning disabilities often have overlapping support and health needs, so we offer a range of flexible residential and supported living services. This ensures every individual has access to the tailored support they need, when they need it.
Our Residential & Support Approach For Learning Disability
Parkfield House will provide care for people living with learning difficulty by ensuring that
- Every service user should have their care reviewed in a face-to-face meeting every 6-12 months, or sooner if there is a change of situation.
- We will work in partnership with the GP practice to support annual reviews of medication and physical check-ups.
- We will work with multi-disciplinary teams to support service users with dementia experiencing deterioration in their care, and those experiencing distressed behaviour.
- We will carry out advance care planning for service users with Learning Disability care needs to ensure that our staff know how to support them and how to recognise any changes in their health at an early stage.


- We will advise and support service users experiencing early onset dementia and/or poor mental health on how to access support groups and voluntary organisations, (including the community drugs and alcohol team, as required).
- We will support service users to self-refer to the local Wellbeing Team through their General Practitioners.
- We will support service users attending the hospital memory clinic with a diagnosis of dementia and who are stabilised on their medication, to promote wellbeing at home in order to avoid frequent visits to the hospital clinic.
- We will work with the local hospitals and General Practitioners to support our service users and to have a system in place to provide timely follow up to their appointments and health needs.
- We will ensure that our staff have a good understanding of how to support service users with dementia needs.
- We will engage with service users with dementia, by enabling them to enhance their sensory abilities and engage more actively in their consultations.
Specialist Support Services
For people with more complex support needs, we can provide greater levels of specialist support. In our comfortable and safe environments, individuals with complex needs can grow their communication, social and life skills in a vibrant, community-based setting.
Each of our homes is carefully designed and adapted to meet any additional needs including autism, sensory needs, complex behavioural needs or health concerns. Our dedicated and highly skilled staff use person-centred approaches and the latest assistive technologies to make the world more predictable and less stressful for the people we support.
